I had to familiarize myself with this for work. I know from a film history class that NC-17 replaced X which was associated with films that were deemed overly sexual explicit (films that feature pornography or excessive frontal nudity, particularly male, have gotten that rating).

Many mainstream theater companies refuse to carry any film rated higher than R. An R rating means someone under the age of 17 can attend with a parent or guardian over 17 (or at least a parent or guardian buying the ticket depending on theater policy). NC-17 means that no one under 17 can be admitted into the theater (NC stands for No Children). Studios will often want to edit content to an R rating simply because it's more commercially viable for distributors (Blue Valentine and Shame are two notable examples of recent films that were released with an NC-17 rating). Ultimately, you can show graphic murders while swearing up a storm, but a high emphasis is placed on sexual content and that can determine the jump from R to NC-17.

I highly recommend Kirby Dick's documentary from 2006 called "This Film Is Not Yet Rated" which is about the MPAA's (Motion Picture Association of America's) rating systems. It's not the best documentary that it could've been in my opinion, but it's informative and highly entertaining.

The MPAA appeals system has been described as hellish. You can't use other films in your defense no matter how recent (i.e. "Well in 'MOVIE X FROM 2016' this happens so how come in my movie ... ") and the board has been described as completely arbitrary in their decisions and guidelines (i.e. number of pelvic thrusts during a scene of sexual intercourse can also determine the difference between PG-13 and R). Dick's documentary is pretty great because it manages to get a lot of filmmakers like John Waters, Kevin Smith, Atom Egoyan, Kimberly Pierce, etc. who have run up against the process.

An example I've found interesting is The King's Speech. A film that is meant to be rather uplifting in its message. Doesn't feature scenes of violence or sexuality (maybe a character smokes a cigarette or drinks alcohol?), but because the character says the F-word more than once during his speech therapy in a very funny scene, the film got slapped with an R rating. From what I remember The Weinstein Co. considered taking it out and did try to appeal the board by emphasizing the film's message, but instead just released it with an R.
