Having my name spelled wrong. Here is a short list of places my name has been misspelled in the past year:

My driver's license
The purchase agreement for my condo
At least half the e-mails either of the attorneys I work for send, even though my name is there, spelled correctly in my e-mail signature every time.

Look, I get it, Eric is the mathematically more common spelling, but Erik is not only very common in its own right, it is also the historically correct spelling. It is a name of Scandinavian origin...it might vary between Erik or Eirik and such, but never Eric.

I think the only person who misspells my name more than my last boss is my wife. I usually don't bother correcting anybody unless it appears in a PPT slide

Most of the time when i tell people my name over the phone they hear it as Jess instead of Jeff.

My boss makes it a point to mispronounce my name by saying it the English way because he thinks it's funny and now most of the people I work with say it wrong too.
