I still like Repulsion way more, since it sits comfortably on my Top 100, but I rewatched Rosemary's Baby somewhat recently and had a much more positive response to it. The annoying neighbor became a really interesting character, and I understood the terrors of living in an apartment, so it made more sense to me. I was also more into the mood Polanski was building. The lack of scares was less worrisome to me. I think I've kinda moved beyond that. I still love a movie that will scare me, but a solid creepy vibe will work almost as well for me.

I recently rewatched it as well. The main 'horror' I felt was just that inevitable dread. Every time she takes a pill or has a smoothie it's just a punch in the gut. The husband turning and then just hounding her time and again leant on me harder and harder.

Both are in my Top 100, Repulsion a bit higher. Don't think the extra scares are the reason, more so the protagonist is a bit more unpredictable. So while I would say scares don't really matter for horror quality, I'm more talking about psychological horror, a sub-category I much prefer to the rest of the genre.
Those are my principles, and if you don't like them... well, I have others.
